Irritable Bowel Syndrome: What Helps

Getting a diagnosis, red flags, dietary approaches and probiotics

IBS is common, genuinely unpleasant and frequently self-diagnosed, which matters because several other conditions produce similar symptoms.

Get a diagnosis first

IBS is diagnosed on symptom patterns after excluding other causes. Coeliac disease, inflammatory bowel disease, bile acid malabsorption and thyroid problems all mimic it, and several are treatable in entirely different ways.

Coeliac testing requires you to still be eating gluten, which is why cutting it out before being tested causes problems.

Red flags that need prompt assessment

Unexplained weight loss, bleeding from the bottom, anaemia, a persistent change in bowel habit over fifty, a lump in the abdomen, or symptoms waking you at night.

These are not IBS symptoms and should be seen by a doctor promptly rather than managed at home.

Dietary approaches

General first-line advice — regular meals, limiting caffeine, alcohol, fat and fizzy drinks, adjusting fibre type — helps many people and is where to start.

The low FODMAP diet has good evidence and is restrictive and complex. It should be done with a registered dietitian and has a structured reintroduction phase, which people frequently skip and then stay unnecessarily restricted.

Probiotics

Effects are strain specific rather than general. Some strains have reasonable evidence in IBS, and a product listing only genus and species cannot be matched to any trial.

Give any product four to eight weeks, then stop if nothing has changed.

Food and symptoms

Keeping a simple diary of food, symptoms, sleep and stress for a fortnight frequently reveals patterns that guesswork misses. Take it to an appointment rather than starting an elimination diet alone.

Eating regularly, chewing properly and not eating late all help more than people expect.

Fibre

The type matters as much as the amount. Soluble fibre such as oats and psyllium suits many people with IBS, while wheat bran commonly makes symptoms worse. Increase gradually with plenty of fluid.

Medication and other options

Antispasmodics, peppermint oil capsules, laxatives and antidiarrhoeals all have a role depending on the predominant symptom, and a pharmacist can advise on which suits. Low-dose antidepressants are sometimes used for gut pain rather than for mood.

None of these should be started long-term without discussing it with a doctor.

Living with it

Symptoms fluctuate, and a bad week is not a failure of management. Planning around flare-ups — knowing where toilets are, carrying what helps — reduces the anxiety that itself worsens symptoms.

Employers should make reasonable adjustments, and a doctor can write in support of that.

The gut and stress

The connection is real. Gut-directed hypnotherapy and CBT both have supporting evidence and are offered through some health services.

Speak to your doctor before starting anything restrictive.
