Doctor-Led Skincare and Clinic Treatments
What topical products can and cannot do, treatments compared, and choosing a practitioner
Skincare sold by clinics sits between cosmetics and medicine, and the honest position is that topical products do some things well and cannot do others at all.
What topicals genuinely achieve
Texture, tone, pigmentation, hydration and a real slowing of photoageing. Retinoids, vitamin C, azelaic acid, niacinamide and above all daily sunscreen have decades of evidence.
Expect eight to twelve weeks before judging anything, since skin turnover takes roughly that long.
What they cannot do
Change the structure of the skin or the tissue underneath. Volume loss, deep static lines, true scarring and laxity do not respond meaningfully to creams, whatever is claimed.
A product promising results equivalent to a procedure is describing a procedure it cannot perform.
Where treatments fit
Microneedling and fractional laser for texture and scarring. Chemical peels for pigment and tone. Injectables for lines and volume. Each needs multiple sessions and has downtime and risks.
Choosing a practitioner
In the UK, most aesthetic procedures are lightly regulated, so who holds the needle matters enormously. Look for a doctor, dentist or registered nurse, with insurance, working from a clinical premises.
Ask what happens if something goes wrong, who manages a complication, and whether they can prescribe the reversal agent where one exists. A practitioner who cannot answer that is the wrong one.
Realistic expectations
Be cautious of anyone recommending a large plan at a first consultation, and of discounts with a deadline attached.
A routine that works
A gentle cleanser, one active, a moisturiser and daily broad-spectrum sunscreen. Adding more products is usually what causes the irritation that people then try to treat with yet more products.
Introduce one thing every four weeks and patch test on the inner forearm for several days first.
Cost
Price correlates poorly with effectiveness in skincare. Well-formulated products at modest prices contain the same evidenced actives at the same concentrations.
Sunscreen specifically
Broad-spectrum, applied generously, every day including winter and cloudy days. Most people apply roughly a third of the amount used in testing, which reduces the protection actually achieved considerably.
Around a third of a teaspoon for the face and neck, reapplied every couple of hours whenever you are in strong sun.
Before and after photographs
Judge any change from photographs taken monthly in the same light and position rather than from memory, since gradual change is effectively invisible from day to day.
Managing expectations
Skin ageing is largely down to genetics and cumulative sun exposure. Products and treatments slow and soften it rather than reversing it.
The basics still dominate
Sunscreen, not smoking, sleep and simple consistency outperform almost everything else on offer.
